by letter, telephone, in person or by submitting the
online form that appears below. Any supervisor can accept
your comments. Your statement will then be forwarded
to the appropriate Captain for review and assignment.
You will be contacted by a supervisor as soon as possible
for consultation and further explanation of the process.
The Online Process

Captain for review. A supervisor will then contact you
to review and clarify your issues. If needed, a complete
investigation will be conducted and you will be provided
with a notification of the results.
